Eligibility
Any health care provider that meets PRIME’s membership criteria is eligible to join. Eligible institutions include but are not limited to:
- Non-profit hospitals
- For-profit hospitals
- Integrated Delivery Networks
- Long-term care facilities
- Assisted living facilities
- Ambulatory Surgery Centers
- Home health care providers
- Clinics
- Hospice
- Institutions (schools, universities, charities)
Procedure
Approval for membership is determined by PRIME after review of the completed application materials. For membership application contact PRIME by:
